Job Description

The Health Risk Assessment (HRA) Supervisor is responsible for the day to day operations of the HRA Program within the Case Management Department. This position oversees the activities to ensure all members who require a Health Risk Assessment receive one in a timely fashion. This position reports to the Senior Manager of Case Management, Steward Health Choice Generations (D-SNP) line of business and interfaces with members, families and physicians as well as health plan staff including Medical Director(s), and other Health Choice personnel as needed.

  • Monitor HRA staff to ensure completion of both initial and annual Health Risk Assessments
  • Assign out all initial and annual HRA’s to staff
  • Communicate and educate on the job expectations to both HRA Nurse Specialists and Care Navigators
  • Ensure new employees are adequately trained and have proper orientation
  • Plan, assign and monitor the HRA process and report results to Senior Manager
  • Monitor calls and documentation completed by the HRA Nurse Specialist and Care Navigator for quality purposes
  • Meet with Senior Manager on a regular basis to assess and discuss program outcomes

Education:

  • Registered Nurse (RN) from an accredited Nursing School

Years of Experience:

  • At least two (2) years nursing experience in a health care setting
  • 2-3 years case management experience preferred
  • 1-2 years of managed care/health plan experience preferred

Specialized Knowledge:

  • Managed Care/Medicare/Medicaid experience
  • Skilled with telephonic Motivational Interviewing
  • Computer and software program skills
  • Written and verbal skills
