Job Description

Position Purpose:

Supports all aspects of the clinical members of the Case Management Team. Manages the flow of referrals and communications from the various sites across the network and outside the network to all the Case Management programs. Serves as a key communication link between patients, referral sources, physicians, patient care facilities and Clinical Staff.

Responsibilities:

Daily creation of all Care Management cases in Care Management EMR

  • Steward Surveillance Trackers
  • Health Information Exchange
  • Health Plan Daily Admission Reports
  • SHCN Care Management Referral phone line and fax line
  • Daily processing of GSMC CHF/COPD report

Faxes and mailings:

  • Distributes patient updates and Discharge Summaries received via the SHCN Fax line to the appropriate care management team member
  • Mails/faxes CCPM action plans to patients and PCPs
  • Mails CHF packets to patients and PCPs
  • Emails Rounder Scorecards to SNF rounders monthly

Reporting requests:

  • Responds to requests to determine if patient is attributed to Steward NGACO
  • Looks up information in Steward Home Care Health documentation system as requested
  • Ad hoc reporting

Administrative responsibilities

  • Mails SNF Rounder Scorecards monthly
  • Schedules Large Group SNF Rounder monthly phone calls
  • Performs targeted follow up calls for ACM
  • Assist with meeting preparation and scheduling including preparation of meeting decks, insertion of data, ensure, font and colors are consistent
  • Assist with orientation schedule for new hires and prepares orientation materials

Education / Experience / Other Requirements

Education:

  • Bachelor Science preferred

Years of Experience:

  • Minimum of two (2) years of health care experience

Work Related Experience:

  • Experience working with referral management in physician’s office or in health plans preferred

Specialized Knowledge:

  • Software/Hardware: Cerner, EMR experience preferred
  • Knowledge of managed care concepts and medical terminology
  • Strong communication and interpersonal skills
  • Proficient with Microsoft Office programs
  • Ability to meet deadlines in high-pressure environment
  • Demonstrates effective interpersonal skills in intense situations
  • Ability to multi task and work with multiple software application
